Family Registration

To join our parish, update your records, or register your child/children for Religious Education, please select either the printable or online version of the form.

If you elect to use the printable form, click the link below and it should open in your browser. From there, using the Menu Bar at the top of your browser window, select File, then select Print. Once printed, please fill out all pertinent fields and mail or bring it to the Parish Office.

If you elect the online method of entry, just scroll down and fill out all pertinent fields. When you get to the bottom, click "Register".
